Class Enrollment Instructor Permission Request

For Instructors

This form is a streamlined process for instructor permission. This Google form takes the place of the pre-pandemic hard copy form that students gave to instructors for signatures.

  1. Students complete the Instructor Permission Request form to request enrollment in a course
  2. The form submissions populate a spreadsheet
  3. The instructor is sent a link to the spreadsheet; you'll either approve or deny the request on the spreadsheet, and add your initials
    • You can add comments in the NOTES section if needed
  4. We'll be notified once you do that, then enroll and notify the student if approved or denied

Please note that we've added this information to the instructor.permission email auto-response, including a link to the form. If a student contacts you asking for permission, please direct them to the form, or give them the link.
